What is community nursing?Community nursing, or district nursing in East Surrey is provided by First Community Health and Care, the provider of NHS community services in the local area. District nurses play a crucial role in local healthcare, visiting people in their own homes or in residential care homes, providing increasingly complex care for patients and supporting family members. Community nurses play a key role locally to keep hospital admissions and readmissions to a minimum, ensuring that patients can return home as soon as possible. The Evening and Night Nursing TeamAn important part of the community nursing service that First Community provides is the Evening and Night Nursing Team. The team provide visits to the patient’s home in the evening and nights. These visits are either: The work includes:
